BEFORE YOUR FIRST VISIT

How we verify your benefits

1

Share your insurance

Upload your insurance card in our secure patient portal during intake.

2

We check your coverage

We confirm your benefits, copay, deductible and any authorization needs.

3

You know your costs

We explain what you are likely to owe before your visit, so there are no surprises.

Your right to a Good Faith Estimate

Under the law, health care providers must give patients who do not have insurance, or who are not using insurance, an estimate of the expected charges for medical services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency services. Ask us for one before you schedule. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

For questions or more information, visit www.cms.gov/nosurprises.

Cancellations

Please give us at least 24 hours notice if you need to cancel or reschedule. If you cancel with less than 24 hours notice or miss your appointment, a $75 cancellation fee will be charged.
